Worker’s Compensation
Your Insurance copay amount is due on your date of service. We will submit your bill directly to your Insurance Company. A bill will be sent to your secondary insurance upon receipt of payment or denial from Insurnace. If you do not have secondary insurance coverage, you will be responsible for any co-pays and deductables at the time of surgery. We must make a copy of each insurance card at the time of registration.
SELF PAY
You will be contacted prior to your surgery with an estimated procedure cost for you surgery. Payment in full must be received prior to surgery. This payment may be adjusted if your surgeon needs to perform a more extensive operation than anticipated.
